help for online feedback system

I'm working on a project in my college to make a Online-Feedback-Management-System for educational institutions.

I'm requesting MODULAR help for creation of
1)Multiple-Choice-Questions with more than 1 Question per page with only one submit button and store the result in database.
2)Report generation based on overall feedback received (both graphical and statistical).
3)Set security-to make the site online only for a specific set of IP address range.

Please Help me out!
